Your Office Visit

 

Please complete the Patient Information and Medical History forms and bring them along with your insurance card(s) to your office appointment.

Any office co-pay will be payable on the day of your appointment.

If you have had a procedure previously, such as a colonoscopy and/or EGD (esophagogastroduodenoscopy or upper scope procedure), with biopsy or polyps, x-rays, CT scan, ultrasound tests or recent lab work, please bring a copy of these reports to your appointment so the doctor can review them.

 

Procedures

 

Please complete the Patient Information and Medical History forms and bring them on the day of your procedure.

Jackson County Gastroenterology currently does not collect any money up front for co-pays or deductibles for any procedures performed by the physicians at the hospital or ambulatory surgery center.

However, money may be due on the day of your procedure. 

The hospital or ambulatory surgery center may require payment on the day of the procedure for any co-pay or deductible. Someone from their insurance verification department may call you to discuss any amounts due on the day of your procedure. You can also verify that information directly with your insurance carrier. You may call them directly to discuss your amount due or any available payment options or payment plans.

If you need to cancel or reschedule your office appointment or procedure please notify our office within 24 hours to avoid late fee.  Numerous canceled or rescheduled procedure appointments will require an office appointment prior to rescheduling.
